
Last week, LonGBoaT Wakefield had the pleasure of meeting with the Live Well Wakefield Team, part of the South West Yorkshire NHS Foundation Trust. The meeting marked the beginning of an exciting partnership aimed at better supporting the health and well-being of LGBT+ adults across the Wakefield District.
About Live Well Wakefield
Live Well Wakefield is a referral-based social prescribing and supported self-management service that provides invaluable support to adults (18+) who either live in the Wakefield District or are registered with a Wakefield GP. Their service recognises the many factors that can impact our ability to feel healthy and stay independent. They offer information, advice, and support for non-clinical needs, helping people address challenges that might otherwise go unacknowledged.
The Live Well advisors and link workers are based in both community and primary care settings across the Wakefield District, offering telephone and face-to-face assessments tailored to individual needs. They maintain a comprehensive directory of social, exercise, and support groups, ensuring they can connect people with the services they need.
Additionally, Live Well Wakefield runs supported self-management sessions to help individuals living with long-term conditions develop the tools and techniques they need to improve their quality of life. These sessions are available online or in person at accessible venues throughout the district. The service accepts both self-referrals and professional referrals. Resources Available at the Hub
As part of this new collaboration, we now have materials from Live Well Wakefield on site at the LonGBoaT Hub. These resources are available for anyone attending our Saturday drop-in sessions (11 AM to 2 PM)Â or any other event hosted at the Hub. We encourage our community members to take advantage of this wealth of information and explore how Live Well Wakefield can support their health and well-being.
